New patient registration

There is a very quick and easy process to register at Storrsdale Medical Centre:

1. Download and fill in our new patient registration form and booklet
2. Bring the completed forms, proof of address and photo ID to reception
3. Our lovely receptionists will put your details on the system and you will now by a STORRSDALE PATIENT!!
